Home Health Nurse – RN or LPN

About the Role

Asheville Home Health is seeking compassionate and skilled RNs and LPNs to join our home health team. As a Home Health Nurse, you will be responsible for providing nursing care to patients in their own homes. You will work closely with patients and their families to develop care plans that meet their specific needs and improve their overall health and wellbeing.

Responsibilities

• Assess patients' health status and develop individualized care plans
• Administer medications and treatments as prescribed by physicians
• Monitor and document patient progress and report any changes to PCP
• Provide education to patients and their families about their conditions and how to manage them
• Collaborate with other healthcare professionals to ensure continuity of care
• Communicate effectively with patients and their families to promote trust and build strong relationships
• Maintain accurate and up-to-date patient records

Requirements & Qualifications

• Active RN or LPN license in good standing in North Carolina
• Minimum of two years of nursing experience preferred; home health experience a plus
• Customer-oriented and friendly
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a team
• Strong critical thinking and problem-solving skills
• Valid driver's license and reliable transportation
